How many bans till perma?

Your first ban could be a perm ban. There are no maximum bans.

Well I think it originally is 10 bans, for normal rulebreakings, before you get a perma.

Back in days I got 9 bans before my permanent one ^^

Small issues like that. Would he be banned permanently?

There's no set limit of bans before a perm. It just depends on how many rules the player broke; and the severity of said violation(s). You could get a perm after 16 Bans, or your very first ban could be a perm.

Well, yes it could be your first ban that gives you a perm. But that would be very unusual.
