This link provides the details you need to fill out the template - http://www.fearlessrp.net/bans/index.php. It is located by clicking on the 'Bans' section at the top right of the forum.
Failure to adequately complete the template means the administrator in question may not be aware of the appeal and also violates courthouse rules, so make sure it is always filled in properly before posting.

Edited the reason, remember to include what is specified on the ban page and not simply your version of events - that can be explained in the "Why we should unban you" section.
