I think it's random, it's not really supposed to happen.

Knife is random as hell, which is why knife fights kinda suck.

I don't think crowbar dmg is real since I've 3 hitted people with it.

Yes
on the chart, it says it can get 3 shot kills when you hit the head
the damage reduces drastically when you hit anywhere else

Yeah I've been a test subject by a crowbar in the past it hits 36 on the head, and about 6-9 on the feet, all depends on where you hit them.
